Abstract

Antibiotic resistance (AMR) is one of the most pressing global public health challenges of the 21st century, threatening the effective prevention and treatment of bacterial infections. It occurs when bacteria evolve mechanisms to survive exposure to antibiotics that were previously effective against them. The emergence and rapid spread of multidrug-resistant (MDR) pathogens have significantly increased morbidity, mortality, healthcare costs, and the burden on healthcare systems worldwide. The major causes of antibiotic resistance include the irrational and excessive use of antibiotics in human medicine, self-medication, incomplete treatment courses, inappropriate prescribing practices, overuse of antibiotics in agriculture and livestock, poor infection prevention measures, and inadequate surveillance. Bacteria develop resistance through genetic mutations and horizontal gene transfer, enabling them to produce enzymes that inactivate antibiotics, alter drug targets, reduce drug uptake, or actively expel antibiotics through efflux pumps. These mechanisms have resulted in the emergence of highly resistant pathogens, including methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant Enterococcus (VRE), extended-spectrum β-lactamase (ESBL)-producing bacteria, and carbapenem-resistant Enterobacterales (CRE).Addressing antibiotic resistance requires a comprehensive, multidisciplinary approach. Key strategies include promoting rational antibiotic prescribing through antimicrobial stewardship programs, strengthening infection prevention and control practices, enhancing public awareness about responsible antibiotic use, implementing robust surveillance systems, encouraging vaccination to reduce infection rates, and supporting research into novel antibiotics, alternative therapies, rapid diagnostic tools, and bacteriophage therapy. Collaboration among healthcare professionals, researchers, policymakers, agricultural sectors, and the public is essential to combat this growing threat. In conclusion, antibiotic resistance is a preventable yet escalating global crisis that demands coordinated action at local, national, and international levels. Through responsible antibiotic use, effective stewardship, continuous surveillance, innovation in drug development, and public education, the spread of resistant microorganisms can be significantly reduced, thereby preserving the effectiveness of existing antibiotics for future generations.
